Create custom field/question during checkout process for specific products.

I would like the ability to add a custom question during the checkout process that only appears for specific products/tickets not changing the global checkout process.
I would like to only ask this question once during the checkout process for our holiday cards product only. People can add as many card versions they want and when they checkout they answer this question only once (instead of every time they add the product to their cart using the current custom field section).
